The 2026 season is almost upon us! Please be aware that the following NEED to be completed/cleared before any particpation in off-season open gyms or when tryouts are held.

1. Physical exam (medical)

2. Consent paperwork

*Must also be academically eligible to play.​

PDF packets are available to download for physical exam and consent paperwork. There is also another PDF packet with information for free of charge physicals at APS/UNM Health Clinics.

*If you have already played on another SHS athletic team this school year, then only the last page needs to be submitted (page 17 Waiver).​
